That being said, knowing a patient’s current cup size and her desired cup size does give me a general idea about how much bigger she wishes to go.
The single most important anatomic measurement I take in consultation is the breast’s base width, which is the horizontal measurement of your breast footprint.
An implant wider than your natural base will sit into the armpit and look unnatural no matter how good the surgery is. That measurement, more than any volume in cubic centimeters, sets the ceiling on what your body can carry well.
Within that ceiling, we choose projection and volume. Volume isn’t the goal; shape is. I always encourage patients not to fixate on the CC’s (cubic centimeters), but rather to think about the implant’s dimensions. This is where sizing systems help. The first week centers on protecting the pocket and managing swelling, and it is shorter and less dramatic than most patients fear. You will wear a supportive surgical bra continuously, sleep with your upper body elevated, and keep your arm activity gentle. I want you walking short distances around the house from the first day, because early movement helps prevent clots and keeps the swelling moving.
Pain is real but manageable. Under-muscle placement hurts more than over-muscle for the first several days because the muscle is being stretched over the implant. I prescribe a non-drowsy muscle relaxer medication to anyone getting her implants under the muscle. When taken round the clock, this medication makes an enormous difference in pain control.
Keep the incisions dry per your instructions, take the antibiotic course as directed, and don’t evaluate your results this week. Everything is swollen, tight, and sitting high. That’s normal, and it is temporary.
