The proposed quote will be valid for 90 days from the date of consultation.
DEPOSIT TO SCHEDULE SERVICES
A deposit may be required at the time of scheduling service. The balance is due at the time of treatment. The deposit is non-refundable if you do not give at least 3 business days’ notice to cancel or reschedule your appointment.
CANCELLATION POLICY
We understand that a situation may arise that could force you to cancel or postpone your treatment. Please understand that such changes affect not only our staff but our other patients as well, and we therefore request your courtesy and concern. If you need to cancel or reschedule your appointment, please allow 3 business days to notify us. Should we receive less than 3 business days of notification, or should you fail to keep your appointment, your deposit may be forfeited.
REFUNDS AND REVISIONAL TREATMENT
There will be no refunds for services already provided. Although good results are anticipated, there can be no guarantee or warranty, expressed or implied, by anyone as to the actual results you may get. Occasionally, additional treatments and/or treatment for problems or complications may be required. These could result in additional charges for which you may be responsible. From time to time as an executive and discretionary measure, fees may be waived to assist patients in achieving their goals, and this in no way indicates an admission of negligence or sub-standard care. Consideration of waived fees for revisions requires you keep all postoperative appointments and that you demonstrate that postoperative instructions were followed.
PRODUCT RETURN POLICY
Returns of non-prescription products will be for exchange only within 15 days of original purchase. All products must be new, unused, and contain original packaging.
